That guide is just a print out of ETKA, most places that you buy the retrofit cruise control from will give you that in an A4 booklet.

Pics are all on Foxy's guide: http://www.foxytech.hostcell.net/control_stalk.htm

The guide is also here: http://uk-mkivs.net/forums/p/221940/1665948.aspx along with discussions about the install.

Loads of people have done this - just do a search and you will find lots of threads about it.

Some engines including mine require an extra loom from the bulkhead to the ECU as the wires are not there. 1J0.971.425.A will be fitted every single time as this is the loom that goes from the cruise control stalk to the bulkhead and fusebox. You will then need the loom 1J0.971.425.B or 1J0.971.425.C (depending on your engine code) to connect the wires that go to the bulkhead, to the ECU.

That was a nice instruction guide in order to really configure the Cruise Control Lever of your Golf MK4. I was kinda amazed that you have gotten that manual or guide. It was indeed a great help for some people here who are in need of those things. Hope you can continue to do these kinds of posts!
